Partnering with Dentsply-Sirona, PrimeOMS leverages Astra Implant System, which has a survival rate of over 98% and a marginal bone maintenance far better than the industrial norm. Certainly, there are less expensive implant systems available, but our primary goal is an implant that does not fail. Astra Implants have proven to provide a level of confidence, consistency, and predictability over and over. When lack of bone is a concern and a patient requires a zygomatic implant, PrimeOMS partners with the global leader, Nobel Biocare, providing a vast array of zygomatic options.
Planning and Guided Surgeries
All PrimeOMS surgeries are pre-planned and “guided”. We utilize a CBCT scan and intraoral impressions to generate a digital representation of the patient’s jaw and oral anatomy. These images are incorporated into our planning software where a final restoration is proposed and reviewed. Upon approval, surgical guides are fabricated for the treatment. The depth, angulation, and mesial-distal and buccal-lingual location of the implant reduction are precisely controlled by titanium sleeves situated within the surgical guides. This provides for accuracy relevant to bone volume, nerves, sinuses, and neighboring teeth. With these components, complete with the patent’s 3D printed jaws, the surgical team can physically review the full treatment pre-surgery. Additionally, guided surgeries are less evasive and have been proven to provide for faster recovery.
PrimeOMS Lab Partner, Oklahoma-based Express Dental Laboratory, is a national leader in digital planning, design, and fabrication of dental restoration products. Express has invested heavily in the digital technologies required to design, mill, and print the various surgical and restorative products required for predictable full-arch restorations. Additionally, their team of in-house knowledgeable experts take personal ownership of each case. Teaming together with Express’s staff on every case from beginning to end insure a consistent, functional, and aesthetically pleasing restoration. Working together, we provide patients with “Crystal Ultra” full-arch restorations. Crystal Ultra restorations are strong, aesthetically pleasing, lighter than zirconia, and, most importantly, repairable at chairside.
PrimeOMS opened its doors working directly with patients to provide “start to finish” full-arch restorations. Most of our direct patients have lost all hope of having a beautiful smile and have not visited a dental office in years. Yet, we have been approached by several general dentists desirous in partnering with us in providing full-arch restoration treatment to their qualified patients. Most general dentists feel completely confident with the restorative stages but are a bit timid on the planning and surgical stages…and rightfully so, it can get complicated, fast. Often many general dentists fall back to what is comfortable and recommend dentures, or partials, or bridges. Therefore, PrimeOMS has created a plan to work with “Selective Referring Dentist”
The Process
PrimeOMS provides your patient with a “Free Consultations” consisting of a CBCT Scan and a pre-treatment plan and pricing. This is not a “dental exam”, as the dentist you already have a history with the patient and know their concerns. The consult is primarily designed to retrieve the CBCT scan to view the full upper and lower arch digitally with the patient. It is to determine bone loss and treatment options. To provide each patient a better educational view of their current condition, their options, and an estimated cost. Following the consultation, our team reports back to the referring office our findings and recommendation. If the patient desires to move forward, we notify the referring dentist and schedule a “Lab-Workup Appointment” in our office. The Lab Workup is where treatment planning begins and where the treatment plan is officially finalized. Upon finalizing the Lab Workup, the treatment planning process begins. This process can take 6 to 8 weeks to complete and is a comprehensive plan from surgery to restorative. During the planning process, the PrimeOMS Planning and Surgical Team will contact the referring dentist to discuss the case as needed. A week prior to surgery, the patient will come to the office for a pre-surgery checkup. During this appointment, a wellness exam is conducted, the patient can ask questions and post-surgery medication is prescribed. Surgery generally lasts 4 to 6 hours, depending on a single or double jaw, plus time for any additional single implants or other procedures. The referring dentist is always welcome to join our team on surgery day. On full-arch immediate load restorations, the patient leaves with their temporary teeth seated and placed on a soft-food diet. The day following surgery, the patient returns for a “Post Surgery Checkup” for evaluations. Any adjustments are made during this appointment. Following the post-surgery checkup, the patent is directed to their referring dentist's office for further checkup and treatment. If slight adjustments are needed, the referring dentist can make those alterations. After the implants fully heal, in 4 to 6 months, the referring dentist works with the PrimeOMS Restoration Team in making final adjustments for the Crystal Ultra arch and/or any zirconia crowns (if there were any single implants placed at the time of surgery).
Cost and Payments
To keep it simple, PrimeOMS bills the patient directly for the surgical stage of the treatment, and the referring dentist bills the patient directly for the restorative stage. PrimeOMS’ standard price for a single-arch restoration (Surgery & Restorative) is $26,500. A full arch restoration is $50,000. The “Surgery Only” price for a standard four-implant single arch is only $12,000 to the referring dentist: $24,000 for a dual arch. (Zygomatic implants, extreme sinus lift, etc. are priced in addition if necessary.) Any additional fees outside the standard fee will be noted during the consult. The restorative stage for a single arch is priced at $14,500 and a dual arch at $26,000. (Although PrimeOMS offers monthly specials to direct patients.) As an PrimeOMS Referring Dentist Partner, your cost for a Crystal Ultra single arch is only $3,800, which includes PMMA try-ins. (Each dental office is responsible for acquiring all necessary restorative tools, such as Astra Torque Wrench, Multi-unit and Hex Driver, screws, and impression copings. Although for the initial patient restoration, these tools may be provided at no charge via the Dentsply-Sirona Representative.)
As each Referring Dentist is providing the restorative services, (adjustments after surgery, removing the surgical temporary, taking a new post-impression, inserting the PMMA, sampling fit, marking the fit, working with the lab on the final design, testing the fit, inserting final, etc.) they are responsible for establishing their own price for the “Final” treatment services. Additionally, they are responsible for communicating this price to the PrimeOMS staff via email. The PrimeOMS team list this price on the pre-treatment plan estimate, along with the referring dentist's name. PrimeOMS is only responsible for collecting the surgical fee from the patient and the referring dental office will collect all fees associated with the “Final” restorative treatment stage. For the patient, there are two separate transactions: surgical and restorative. If the Dental Offices prefers, PrimeOMS is willing to collect payment in full and then pay the referring dentist at the time the final treatment begins. The referring office is also responsible for setting up an account with Express Dental Laboratory and maintaining the account in good standings.
Getting up to Speed
PrimeOMS provides CE Credit courses to insure our “Selective Preferred Dentist” gets up to speed fast. Courses are provided virtual and in classroom and seminar settings. Additionally, the PrimeOMS’ Restorative Team will join the dentist during their first case to review critical points, provide support virtually via phone and video conferencing. Furthermore, preferred dentists are invited to participate in surgeries.
